Need a name tag?

If you need a new name tag please see one of the MLC Supervisors in person in the MLC Admin office in the basement of the South Lodge or use the form below.

  • Examples are First Name Last Name. Or just nick name. For privacy reasons you may omit your last name and just use your initial (e.g. Heidi S.)
  • Your pronouns will be added to the end of your preferred name like this: Kelly Skiaslot (he/him). Terry (she/her). Juniper (they/them)

Most aspects of the Instructor Portal do NOT require you to be logged in. Try accessing the info you seek before trying to log in.
