You can change your schedule at any time.

If you have:

• a non-illness/injury related Call In
• AND, it’s more than 72 hours before your requested schedule change

Use the Schedule Change Request Form

When submitting a schedule change request, you will need to let us know the dates you need off and the dates you plan to substitute for the changed dates if there are part of your schedule commitment. If the days you choose to replace your scheduled days are not days when we need your help, you may need to choose a different make up day.

This form is also used for “Picking Up Shifts” and “Deleting Extra Shifts”
